RESUMO

The paper shows a role of environmental objects in the formation of parasitic diseases in Belgorod-Dnestrovsk, developmental changes in intestinal parasitic cenoses in children from closed institutions. To attract parasitologists to the diagnosis of acute intestinal infections of unspecified etiology increases the percent (49.5 to 82.0) of positive results. The findings show what environmental objects should be monitored in the sanitary and parasitological contexts: soil, vegetables, greens in helminthiasis; utensils in preschool children' establishments in contact helminthiasis; drinking water, vegetables, greens in giardiasis.

RESUMO

The authors submit results of treatment of 30 patients presenting with neuroendocrine-metabolic form of hypothalamic syndrome with a 3-months course administration of nifedipine on a 30-mg daily schedule. Before the treatment, the patients had persistent arterial hypertension and abnormal values for central hemodynamics presenting as hypokinetic syndrome. After the treatment with corinfar, the patients fared significantly better, with the arterial pressure and index of the general peripheral vascular resistance getting down. Values for minute volume, stroke volume, cardiac index tended towards normalization.

RESUMO

Atrial natriuretic peptide is shown to be a hormone having a certain part to play in the regulation of arterial blood pressure and water-salt metabolism. Results of study on the plasma content of the hormone in patients with various forms of arterial hypertension of endocrine genesis are presented. Differentially directed changes have been revealed in concentrations of immunoreactive atrial natriuretic peptide. Elucidation of the role of these in the pathogenesis of the pathology under consideration warrants further study in a comparative evaluation wtih the parameters of cardiohaemodynamics and water-salt metabolism.

The content of basal immunoreactive atrial natriuretic hormone (ANH) in the venous blood plasma was studied in patients with various types of endocrine hypertension (Conn's syndrome, pheochromocytoma, neuroendocrine form of hypothalamic syndrome, diabetes mellitus) and diffuse toxic goiter. Increase of the ANH level was revealed in patients with primary aldosteronism, diabetes mellitus and hypertension, as well as with decompensated thyrotoxicosis and pronounced cardiac changes. In patients with neuroendocrine pattern of the hypothalamic syndrome the ANH level remained unchanged, in those with pheochromocytoma it decreased.

Blood contents of pressor peptide hormones vasopressin and angiotonin II were studied in patients with neuro-endocrine syndrome before and after single intake and prolonged treatment with anti-serotonin drug peritol and cholinergic agent parlodel which affect biogenic amine metabolism and, consequently, influence blood pressure. Single doses of the drugs were established to cause different blood dynamics of vasopressin and angiotonin II which classified as marked and paradoxic reactions on peritol and parlodel used separately and associatively. Fall of blood vasopressin content induced by single dose of parlodel was accompanied by blood pressure decrease. Tree-week treatment with peritol and parlodel exerted hypotensive effect and significantly reduced vasopressin blood content.

RESUMO

Thyroid cancers in children from the affected areas of Ukraine were investigated by means of light (33 cases) and electron (20 cases) microscopy. Statistical data from 25 regions of Ukraine have been analyzed concerning thyroid cancer in patients aged up to 18 years for the period 1981-1990. There is an increase in the number of thyroid cancers in the children of Ukraine from 0.04-0.06 cases in the "pre-chernobyl" period to 0.23 cases per 10(5) children's population in 1990. In the children operated in the Clinic of the Institute of Endocrinology in 1990-1991, severe forms of thyroid lesions with metastases to the lymph nodes were noted. Histopathological studies show prevalence of different variants of papillary cancer with presence of follicular, solid, poorly differentiated areas. Ultrastructural analysis revealed polymorphism of tumour cells, various degrees of their differentiation and functional specialization that point to a mixed multicomponent nature of carcinomas.

RESUMO

The paper is concerned with the results of a radioimmunoassay of the state of the renin-angiotensin-aldosterone system in patients with a neuroendocrine-metabolic type of the hypothalamic syndrome. They received pathogenetic therapy with peritol (an antiserotonin drug) or parlodel (a dopaminergic drug) during three months and routine therapy. The latter was not enough to return to normal the basal blood levels of aldosterone, reaction of the glomerular zone of the adrenocortical substance and the juxtaglomerular apparatus of the kidneys. Peritol therapy normalized the blood concentration of aldosterone; reactions of the glomerular zone of the adrenocortical substance and the juxtaglomerular apparatus to insulin hypoglycemia and furosemide acute testing improved considerably. After a course of parlodel therapy blood concentration of aldosterone got to normal, and reaction of the glomerular zone of the adrenocortical substance and the juxtaglomerular apparatus to insulin hypoglycemia was unchanged whereas it improved after furosemide testing.

RESUMO

The article deals with the analysis of works of S.S. Yudin's students and followers devoted to scientific practical and scientific-organizational research into optimization of infusion-transfusion therapy in emergency service hospitals in emergency states: multiple and combined trauma, the crush syndrome, burns, exogenous poisoning, and pyoseptic complications in surgical and traumatological practice. The principles of transfusion media rationing in an emergency service hospital, particularly in mass admission of patients and accident victims, are substantiated. The medico-economical efficacy of including infusion-transfusion therapy in complex treatment of emergency conditions is shown.

Blood vasopressin concentration, hypothalamic response to stress resultant from insulin hypoglycemia and to acute furosemide load were measured in 72 patients with neuro-endocrine-metabolic form of hypothalamic syndrome. Pathogenetic treatment was decided upon by sensitivity to dopaminergic drug parlodel and antiserotonin drug peritol. According to the sensitivity tests the patients received either parlodel (5 mg/day) or peritol (12 mg/day) for 3-6 months. There were also patients on symptomatic treatment aimed at reduction of body weight. Peritol treatment promoted a decline in basal blood level of vasopressin and better response to insulin hypoglycemia and furosemide test. Parlodel treatment normalized vasopressin blood concentration and hypothalamic response to stimulators. Routine symptomatic therapy did not induce differences in vasopressin level compared to active stage of the disease.

Thyroid function was studied in 38 patients with diffuse thyroid goiter after subtotal thyroidectomy (3.82 and 25.9 months after surgery. Postoperative hypothyroidism developed less frequently in diffuse goiter patients who showed before treatment thyro-stimulating antibodies in the blood. The authors recommend a differential approach to the volume of thyroidectomy depending on the presence and absence of thyroid-stimulating antibodies before the operation.

The informative capacity of various statistic parameters characterizing the ecology of pathogens of purulent processes was studied. Many-years' results of the routine bacteriological tests of the materials from purulent wounds in patients from surgical, traumatological and burn departments served as the study subjects. The frequency, persistence and contamination capacity of the pathogens were investigated. Data on the changes in the frequency parameter with respect to the main pathogens of wound infections in various contingents of patients were collected. In relation to the observation period and contingent of the patients, various microorganisms were found to be permanent, supplementary and casual. The use of the ecological parameters was shown to provide integrated information on the etiological role of the pathogens in the development of purulent processes. The information characterized the microbial properties which should be considered in planning the use of antibacterial drugs and estimating the efficacy of the epidemiological control.

RESUMO

The composition of the causative agents of suppurations, isolated from suppurating wounds of patients hospitalized at different departments of the Sklifosovskii Emergency Aid Institute was studied. The proportion of representatives of different microbial families among more than 18,000 strains of the causative agents of wound infection was estimated. Fluctuations in the amount and structure of the agents were analyzed in different clinics over the period of 1967-1987. Under the conditions of emergency aid hospital Staphylococcus aureus proved to be the most frequent causative agent of suppurative inflammatory processes at different departments. At the same time no tendency towards an increase of the etiological role of any microbial family in the development of wound infections was noted. The incidence of different causative agents of suppurations was found to vary at different departments, depending on their specialization and season. The results of observations on the composition of the causative agents of wound infection may be used for the planning and evaluation of antiepidemic measures.

The paper is concerned with the results of investigation of the blood concentration of alpha-melanocyte-stimulating hormone (alpha-MSH), hypophyseal reactivity to stress caused by insulin hypoglycemia in 26 patients with diseases of the hypothalamo-hypophyseal-adrenocortical system (Itsenko-Cushing disease and syndrome, hypothalamic syndrome of the pubertal period). A radioimmunoassay was used for the determination of the blood levels of alpha-MSH, ACTH and cortisol. In healthy persons the blood level of alpha-MSH rose 2-fold as compared to the basal one under the influence of insulin hypoglycemia-induced stress. In patients with Itsenko-Cushing disease at the active stage, alpha-MSH blood concentration was significantly elevated and a hypophyseal reaction to stress was disturbed. In Itsenko-Cushing syndrome, a decrease in the blood basal level of alpha-MSH and a hormone reaction to insulin hypoglycemia was noted. In patients with the hypothalamic syndrome of the pubertal period the blood level of alpha-MSH was raised and hypophyseal reactivity to insulin hypoglycemia was changed. Correlation analysis of the levels of alpha-MSH, ACTH and cortisol showed direct noticeable close relationships between the level of alpha-MSH and ACTH concentration in healthy persons and direct high close relationships between these indices in patients with Itsenko-Cushing disease. Correlation between the blood levels of alpha-MSH and cortisol was undetectable.
